Abstract: This article reports a rare case of a 42-year-old male diagnosed with giant mesenteric cavernous lymphangioma (MCL) in adulthood. The patient was admitted to the hospital due to episodic abdominal pain.Imaging examinations (enhanced CT and color Doppler ultrasound) revealed a cystic lesion with clear boundaries and no enhancement, measuring approximately 11 cm×6 cm×9 cm in size, located in the right lower abdomen, which is consistent with the typical characteristics of MCL.Due to the large tumor size and the resulting compression symptoms, there were clear surgical indications.During the operation,it was found that the tumor was closely related to the mesentery and blood vessels, with mesenteric vessels running through the tumor, making it impossible to simply dissect them. Therefore,a combined mesenteric lesion resection and intestinal resection and anastomosis was ultimately performed.Postoperative pathology and immunohistochemical results confirmed the diagnosis of cavernous lymphangioma. The patient recovered well after surgery, was discharged smoothly, and showed no recurrence during short-term follow-up.
