机器人辅助腹腔镜下结肠代输尿管技术在儿童上尿路修复与重建手术中的应用

Clinical research of robotic-assisted colonic ureter in the treatment of upper urinary tract repair and reconstruction in children

摘要: 目的 探讨机器人辅助腹腔镜下结肠代输尿管技术在儿童上尿路修复与重建手术中的临床效果。方法 2017年7月~2024年6月解放军总医院第七医学中心收治的机器人辅助腹腔镜下结肠代输尿管技术治疗输尿管长段缺损患儿20例。所有患儿术前均行泌尿系 B 超、磁共振泌尿系水成像、利尿性肾核素显像及排尿膀胱尿道造影等检查明确诊断,部分患儿行经尿道逆行输尿管造影检查。所有患儿均采用机器人辅助腹腔镜下结肠代输尿管技术修复长段输尿管缺损。在进行肠管代替输尿管缺损时采用3种不同处理方式:8例采用Yang-Monti结肠代输尿管术替代输尿管,8例采用带蒂结肠补片技术(Onlay技术)替代输尿管,4例采用改良YangMonti结肠代输尿管术替代输尿管。术后并发症采用 Clavien-Dindo分级进行评估。结果 所有手术均在机器人辅助腹腔镜下顺利完成,无严重术中并发症,无中转传统腹腔镜或开放手术。平均输尿管缺损长度(6.10±2.13)cm,平均手术时间(308.0±24.5)分钟,术中平均出血量(52.5±27.5)ml,平均开始口服喂养时间为(5.35±0.75)天,术后平均引流管留置时间(5.8±1.0)天,术后平均住院时间(9.3±1.63)天。8例患儿术后出现Clavien-Dindo Ⅰ~Ⅱ级并发症,均给于保守治疗。3例患儿术后出现Clavien-Dindo Ⅲ级并发症,均系双J管堵塞导致患侧肾积水增加,更换支架或肾造瘘后好转。所有患儿术后8周左右取出体内支架管,同时行患侧输尿管镜检查,其中19例吻合口愈合良好,管腔通畅,1例替代段输尿管局部管壁毛糙、部分粘连,行球囊扩张1次。平均随访时间(42±17)个月。所有患儿术后复查泌尿系超声检查提示患侧积水较术前减少或保持稳定,利尿性肾动态显像提示肾功能均有不同程度的改善,无明显梗阻。结论 机器人辅助腹腔镜下结肠代输尿管术修复儿童长段输尿管狭窄或缺损安全可行,为治疗儿童复杂上尿路修复或重建手术的终极治疗方案提供了一种新的选择。

Abstract: Objective To investigate the clinical efficacy of robotic-assisted laparoscopic colonic ureter in the repair and reconstruction of the upper urinary tract in children.Methods We retrospectively analyze the clinical data of 20 children with long-segment ureteral defects who underwent robotic-assisted laparoscopic ureteral replacement surgery at The Seventh Medical Center of PLA General Hospital from July 2017 to June 2024.All children underwent preoperative examinations,including urinary system ultrasound,magnetic resonance urography,diuretic nephrolinoscintigraphy and voiding cystourethrography,while some patients underwent retrograde ureteral catheterization.All children were treated with robotic-assisted laparoscopic colonic ureteral replacement to repair the long-segment ureteral defects.Three different techniques were employed for the colonic replacement:8 cases used the Yang-Monti colonic ureteral replacement,8 cases used the pedicled colonic patch technique (Onlay technique),and 4 cases used the modified Yang-Monti colonic ureteral replacement.Postoperative complications were evaluated using the ClavienDindo grading system.Results All surgeries were successfully completed under robotic-assisted laparoscopy without severe intraoperative complications or conversion to traditional laparoscopic or open surgery.The average length of ureteral defect was (6.10±2.13)cm,the average operation time was (308.0±24.5) minutes,the average intraoperative blood loss was (52.5±27.5)ml,and the average time to start oral feeding was (5.35±0.75) days.The average indwelling time of the drainage tube after the operation was (5.8±1.0) days,and the average postoperative hospital stay was (9.3±1.63) days.Eight children developed Clavien Dindo Ⅰ-Ⅱ complications,all of which were managed conservatively.Three children experienced ClavienDindo Ⅲ complications,all due to double-J stent obstruction leading to increased hydronephrosis on the affected side,which resolved after stent replacement or nephrostomy.All children had their internal stents removed approximately 8 weeks postoperatively,and ipsilateral ureteroscopy was performed simultaneously.Among them,19 cases showed well-healed anastomoses with patent lumens,while 1 case exhibited localized roughness and partial adhesion of the replaced ureteral segment,which was treated with one session of balloon dilation.The mean follow-up duration was (42±17) months.Postoperative urinary system ultrasound showed reduced or stable hydronephrosis on the affected side compared to preoperative levels,and diuretic renal scintigraphy revealed varying degrees of improvement in renal function,with no significant obstruction.Conclusion Robotic-assisted laparoscopic colonic ureteral replacement is a safe and feasible approach for repairing long-segment ureteral strictures or defects in children.It provides a novel option for the ultimate treatment of complex upper urinary tract repair or reconstruction in children.
