Fecal microbiota transplantation for comorbid depression and constipation in adolescents:therapeutic strategies and future perspectives

Abstract: The comorbidity of depression and constipation in adolescents has emerged as a critical public health challenge.These two conditions interact bidirectionally via the gutbrain axis,thereby significantly exacerbating the overall disease burden.Conventional therapies frequently fail to overcome the therapeutic bottleneck of this comorbidity due to their limited efficacy and potential gastrointestinal adverse effects.Driven by the core pathophysiology of gut microbiota dysbiosis,fecal microbiota transplantation (FMT) offers a novel approach to concurrently ameliorating mood disorders and restoring intestinal function by reestablishing microecological homeostasis.Although FMT in the neuropsychiatric domain remains in the clinical investigational phase,it is rapidly evolving toward standardized and personalized clinical paradigms.This transition is profoundly accelerated by artificial intelligence (AI)assisted donor screening,multiomicsdriven precise colonization,and the optimization of standardized preparation protocols.Ultimately,this strategy holds great promise for overcoming the therapeutic hurdles of refractory comorbidities,providing adolescent patients with a safe,durable,and novel microbiotatargeted therapeutic regimen.
