Abstract: Fecal microbiotabased therapy,which treats diseases by restoring intestinal microbiota balance,is an emerging strategy that has attracted considerable attention in gastrointestinal surgery and related fields.The American Gastroenterological Association(AGA) recently issued clinical practice guidelines focusing on fecal microbiota transplantation(FMT) and two U.S.Food and Drug Administration(FDA)approved commercial microbiotabased preparations.These guidelines propose seven core recommendations for managing recurrent Clostridioides difficile infection,inflammatory bowel disease,and irritable bowel syndrome.This article systematically reviews the key recommendations of the guidelines,discusses the indications,methodologies,efficacy,and safety evidence supporting such therapies across different conditions,and analyzes current controversies and future research directions,with the aim of providing a scientific reference for surgical and gastroenterological clinical practice.
