The current situation and prospects of nanoknife therapy for locally advanced pancreatic cancer

摘要: 胰腺癌起病隐匿,侵袭性强,预后极差。目前,手术切除仍然是延长胰腺癌病人生存的主要治疗手段,但多数病人确诊时已失去手术机会。不可逆电穿孔(IRE)是一种非热能消融技术,不仅可以选择性局部消融肿瘤,还能够逆转高度免疫抑制的微环境,联合化疗和免疫治疗有望获得更好的临床疗效。本文系统综述了IRE联合化疗与免疫治疗的最新证据,重点剖析了IRE重塑胰腺癌免疫抑制微环境的潜在机制,并探讨了优化联合治疗策略的未来方向,旨在为胰腺癌的治疗提供新的视角和理论依据。

Abstract: Pancreatic cancer is insidious in onset, highly aggressive, and associated with an extremely poor prognosis.Currently, surgical resection remains the primary means of prolonging survival in patients with pancreatic cancer; however, the majority of patients lose the opportunity for surgery at the time of diagnosis.Irreversible electroporation (IRE) is a non-thermal ablation technique that not only achieves local tumor ablation but also reverses the highly immunosuppressive tumor microenvironment.The combination of IRE with chemotherapy and immunotherapy is expected to yield improved clinical outcomes.This paper systematically reviews the latest clinical evidence on IRE combined with chemotherapy and immunotherapy, with a focus on the potential mechanisms by which IRE remodels the immunosuppressive microenvironment of pancreatic cancer.It also explores future directions for optimizing combination treatment strategies, aiming to provide new perspectives and a theoretical foundation for the treatment of pancreatic cancer.
