基于LncRNA表达的胃癌病人术后早期复发风险预测模型构建分析

Construction and analysis of early postoperative recurrence risk prediction model for gastric cancer patients based on LncRNA expression

摘要: 目的 构建基于长链非编码 RNA(long non-coding,LncRNA)表达的预测模型,并分析其在预测胃癌病人术后早期复发风险中的预测效能。方法 前瞻性纳入2016年6月~2019年1月于本院行胃癌切除术的病人121例,术后通过门诊或电话随访2年,根据病人随访情况,将病人分为复发组和未复发组。基于胃癌术后复发基因组图谱测量病人lncRNAs,使用 edgeR 筛选的差异表达的LncRNA(differentially expressed long non-coding RNAs,DEL),采用COX 和Lasso-Logistic回归方程筛选与早期复发相关的LncRNA,应用ROC曲线来评估模型的有效性。结果 术后随访2年,共失访21例,最终纳入100例病人,其中31例病人出现复发为复发组,69例未复发的病人为未复发组。在胃癌术后病人中,edgeR 共筛选出992个DEL,其中772个上调,220个下调。单变量分析显示,复发组与未复发组63 个 lncRNA 表达存在差异;Lasso-COX 回归模型分析显示,22个 lncRNA 是胃癌病人术后早期复发的风险因素;将22个 lncRNA 进一步纳入多变量 COX 回归分析,结果显示,AC007991.4、AC079385.3和AL109615.2是胃癌病人术后早期复发的强风险因素,三者联合预测胃癌病人术后早期复发的AUC为0.879。结论 AC007991.4、AC079385.3和AL109615.2是影响胃癌病人术后早期复发的因素,构建风险预测模型,可指导重点人群检测。

Abstract: Objective To construct a prediction model based on the expression of long noncoding RNA (long non-coding,LncRNA) and analyze its predictive power in predicting the risk of early postoperative recurrence in patients with gastric cancer.Methods A total of 121 patients who underwent gastric cancer resection in our hospital from June 2016 to January 2019 were prospectively enrolled.They were followed up by outpatient clinic or telephone for 2 years after surgery.According to the follow-up of patients,the patients were divided into recurrence group and non-recurrence group.Group.The lncRNAs of patients were measured based on the genomic map of postoperative recurrence of gastric cancer,the differentially expressed long non-coding RNAs (DEL) were screened by edgeR,and the LncRNAs associated with early recurrence were screened by COX and Lasso-Logistic regression equation,and the ROC curve was applied.to evaluate the effectiveness of the model.Results After 2 years of follow-up,a total of 21 patients were lost to follow-up,and 100 patients were finally included,of which 31 patients had recurrence as the recurrence group,and 69 patients without recurrence were regarded as the non-relapse group.In patients with gastric cancer surgery,a total of 992 DELs were screened by edgeR,of which 772 were up-regulated and 220 were down-regulated.Among them,univariate analysis showed that there were differences in the expression of 63 lncRNAs between the recurrence group and the non-recurrence group;Lasso-COX regression model analysis showed that 22 lncRNAs were risk factors for early postoperative recurrence of gastric cancer patients;22 lncRNAs were further included in the multivariate COX Regression analysis showed that AC007991.4,AC079385.3 and AL109615.2 were strong risk factors for early postoperative recurrence of gastric cancer patients,and the AUC of the three combined to predict early postoperative recurrence of gastric cancer patients was 0.879. Conclusion AC007991.4,AC079385.3 and AL109615.2 are factors that affect the early recurrence of gastric cancer patients after surgery.The construction of a risk prediction model can guide the detection of key populations and prolong the survival time of patients as much as possible.
