Abstract: Arterial involvement by pancreatic cancer remains a major obstacle to curative resection, and the role of arterial resection (AR) and reconstruction in pancreatic surgery has long been controversial.With the ongoing optimization of neoadjuvant therapy regimens and the attendant improvement in oncological outcomes, coupled with the overall enhancement of perioperative safety in pancreatic surgery, the value and indication of AR in borderline resectable and locally advanced pancreatic cancer warrant reappraisal.This review summarizes the historical controversies surrounding AR and, drawing on recent retrospective studies from highvolume centers and metaanalyses, evaluates its perioperative safety and oncological efficacy and discusses the technical feasibility of AR and the application of arterial divestment(AD) as an alternative strategy.
