Abstract: Objective To evaluate the clinical value of interventricular shunt—a novel approach that avoids the implantation of a shunt pump and peritoneal catheter—as a salvage treatment for trapped temporal horn. Methods A retrospective analysis was conducted on two patients with trapped temporal horn who underwent interventricular shunt after failed endoscopic treatment. The efficacy of the procedure was assessed based on clinical and imaging outcomes. Results Two patients received ventricular shunt two years after the operation. The muscle strength of the left limb of Case 1 improved from grade 2 to grade 5,and the headache of Case 2 completely disappeared. The temporal Angle widths at the same level on the affected side of the two patients decreased by 32.5% and 51.1% respectively compared with those before the operation, and the modified Rankin Scale (MRS) scores of both improved from grade 4 to grade 0.No complications related to the interventricular shunt device occurred during the follow-up period. Conclusion For patients with trapped temporal horn complicating resection of lateral ventricular trigonal tumors, interventricular shunt represents an effective individualized salvage option when endoscopic intraventricular adhesiolysis yields suboptimal outcomes.
