Abstract: Congenital diaphragmatic hernia (CDH) in adults is rare and often misdiagnosed, leading to delayed treatment.A 29-year-old male patient was admitted on November 21,2024,with progressive dyspnea.CT imaging revealed a left diaphragmatic defect with massive herniation of abdominal organs into the thoracic cavity. On the sixth day of hospitalization, the patient underwent laparoscopic tension-free repair of the left diaphragmatic hernia.Intraoperatively,a 5cm diaphragmatic defect was observed, and the herniated contents were successfully reduced into the abdominal cavity and secured with a mesh. The postoperative recovery was good, with full lung re-expansion and no significant complications. No recurrence was observed during the 3-month follow-up.
